How does Petplan insurance work?

Petplan insurance reimburses pet owners for veterinary fees incurred in treating an accident or illness. Petplan insurance works according to a simple 7 Step process:

• 1. Purchase one of Petplan's policies either on-line or over the telephone.
• 2. Visit your current veterinarian or any veterinarian you choose and pay for your veterinary treatment as you normally would.
• 3. Obtain a claim form from Petplan (either by downloading it from the web-site, telephoning or e-mailing us to request one to be sent to you)
• 4. Have your veterinarian sign and date the Claim Form.
• 5. Attach the original receipt from the veterinary clinic to the Claim Form and mail it or fax it to us.
• 6. After receiving the required documentation Petplan will then process the claim.
• 7. A check for the amount covered by your policy will be sent to you.

How do I change the personal information on my policy, if I were to move for example?

If you need to change any of your personal information on your policy you need to inform us in writing either:

• By mail to Customer Care, Petplan, 1 International Plaza, Suite 140, Philadelphia, Pa 19113
• By email to CustomerCare@GoPetplan.com
• By fax to our Customer Care Department at 866.599.4654.

Don't forget to include your policy number, name, and telephone number whenever corresponding with us. This allows us to confirm that we are changing the account on the correct policy.
